Community Cinema Club Privacy Agreement

    As the participant is a minor, parental or guardian consent is required to process this registration. By submitting this form, the participant confirms that consent has been obtained from a parent or guardian.

  6. Media Release Consent
    By attending this event, you may be photographed or recorded as part of the event’s promotional activities.
